I appreciate the historic view of what passed in the House last evening. It seems to be more of "insurance reform" than "health reform." Let's hope its but a start to achieve "health" reform. As Voltaire said, we shoudn't let the perfect be the enemy of the good. Of course, he also said, "a witty saying proves nothing."

Nice summary of the healthcare environment today, Tim. As everyone these days bears down on "fixing this broken system", it's important to recognize who has been holding the broken pieces together all these years.

The reviews of his speech seem to depend on party affiliation - inspiring for the Dems, combative for the Repubs. Which tells me that politics still plays too deep a role in an issue that would be better served by data and policy expertise (which we count on from Tim!).

I'm happy to see you highlight reimbursement - I wish people would drop the banalities about a "public option" and instead discuss the relevant points therein - reimbursement rates, paperwork, the cost of such a program if the employers dump their pool into it, etc.

Let's hope, at least for a minute, for civility over acrimony. Just announced that Sen. Harkin of IA will lead Senate HELP committee, another source of potential safeguard for rural providers in any reform effort. Keep up the good work there at RWHC.

Excellent input. I am an RN RD MPH who completed an FNP with a focus on vulnerable pop care. Rural settings need to be placed in another category.
#1 RNs & Med Students from these areas need to be assured 20% of the admit base
#2 We need to assure that these students received priority ARRA funding to repay loans.
#3 We must use GIS tools like that at the University of MS & demand that rural groups receive telemedicine training urgently. That is a percentage of the community based participatory research & comparative efficacy reform for larger metro HC universities must be related to this.
#4 We must use our APNs & PAs effectively by prioritizing opening new programs in communities with smaller population density. These communities represent 56% of the land mass so that it only equitable.
#5 With the Telemedicine Associations we should demand that expert panels be brought in to give rural areas expertise in health IT. Examples for this type of integrate program include Purdue University School of Nursing combined with their engineering program.
#6 We should state that if 20% of admits to schools are for rural citizens, this must include all schools so that rural areas obtain information transfer at a rate which is consistent with their urban partners
#7 We should work with the largest NGO research team---> The Gates Foundation to assure that vulnerable rural American children are prioritized
#8 We should demand that an investigator team for the Children's Health Study propose interventions annually from data streams. Action is needed not further arcane proposals. We know the maxims but we need the manpower.

Thanks for reading. My angst is related to seeing too many rural children go with inadequate care.

It's interesting to note that the existing reform proposals do little to control costs (at least meaningfully), which means that can will be kicked down the road for another day. You're exactly right, a proactive rural advoacy position while we wait for the other shoe to drop (cost controls) is precisely what we need!
